When a large organization fails, it typically fails suddenly and quickly.  Sometimes the failure comes with minimal warning, like the banking collapse of 2008.  The more likely scenario involves a gradual decline that precedes a quick collapse based upon some big event that sends it over the cliff edge – think the auto industry in 2008.  Such gradual declines are often marked with a number of bad product decisions, under- or malinvestment by the organization, and an internal crisis of confidence in the organization's leadership that leads to the best and most prescient employees taking off for other more successful firms before the ultimate collapse occurs.  These problems can be especially acute for organizations that were once successful and are trying to rediscover their prior glory.  These general patterns hold whether it's a public business, a non-profit charity, or even an English soccer club.
